AFC Services Support Home-based Caregiving

Charles River Center’s offers Adult Family Care (AFC) provides financial and administrative supports for families caring for a loved one with disabilities in their home in Greater Boston’s Suffolk, Middlesex, Norfolk counties. AFC (also called Adult Foster Care) is designed to support people ages 16+ with disabilities who cannot safely live alone due to their medical, physical, cognitive or psychiatric difficulties.

AFC supports individuals living at home who require a full-time caregiver to assist with:

Hands-on personal care assistance for bathing, toileting, dressing, eating, mobility and other activities of daily living (ADLs)
Cueing and supervision throughout an entire task pertaining to the member’s (ADLs)
Behavioral management and supports

We tailor our AFC supports to your unique needs and preferences, and honor and value cultural diversity. Our experienced team of case managers and registered nurses (RN) partners with your family to help you care for your loved one safely, in the comfort of your home.

Program Highlights

Supports available to family caregivers and their loved ones includes:

A tax-free monthly stipend (tiered, based on level of support needed)
Regular support visits (monthly or bi-monthly) from a CRC nurse and a case manager, based on the need level determined by MassHealth
Referrals to other programs and services at CRC and other organizations that provide additional supports to both the caregiver and AFC participant

Eligibility

The individual receiving care must:

Live with a caregiver in a private home or apartment in Greater Boston
Be 16 years or older
Require hands-on, full time assistance with activities of daily living, personal care, and behavioral supports
Qualify for MassHealth (Standard or Common Health) or a version of a Senior Care Options (SCO)
Have a doctor’s order showing a medical, mental, and/or physical need

To be eligible for our AFC program, a caregiver must:

Be at least 18 years old
Live with the person who needs assistance
Caregivers can be family members, but cannot be spouses or legally responsible relatives
Pass a CORI (criminal offender record information) check
Follow AFC regulations and requirements, including home visits
Registration and intake required
