In-home Supports for Living Independently

Individual in-home support services from Charles River Center enable adults with disabilities to maximize their independence while living in their own apartment in Greater Boston, Middlesex and Norfolk counties. Maintaining one’s own household is a valued component of independent living and Charles River Center’s in-home support services support this goal.

CRC’s team of experienced Case Managers provide flexible support and training, depending on the need of each individual. Following an intake, CRC staff work with the individual to create an ISP support plan that enhances their skills and ability to live independently. Goals can be short term or long term.

Program Highlights

Assistance is provided for:

Money management, budgeting and bill paying
Medical and dental care appointments and prescriptions
Food shopping, cooking
Household upkeep and laundry
Transportation as needed
Psychological and social work consultants as needed
Typically provided for 6 -14 hours per week

Eligibility

Contact your DDS Service Coordinator to request in-home supports.
Based on the DDS assessment of the individual’s needs, they will make a referral to Charles River Center for in-home living supports.
